But when Belgian chemist Paul Janssen created it in 1959 as an anesthetic and pain reliever, fentanyl was not destined to become the 21st century&#8217;s most widespread and deadly drug.<\/p>\n\n\n\n<p>Even very small amounts of it can cause respiratory depression, leading to death. A very short period of use is enough to cause addiction.<\/p>\n\n\n\n<p>Cheap as a synthetic opioid, it is now commonly used to adulterate heroin, cocaine and other powdered drugs. But in drug markets, it even comes in pills, which look like prescription painkillers and anxiolytics, but are just like fentanyl.<\/p>\n\n\n\n<p>They are mass-produced in illegal drug cartel laboratories, mainly in Central and Latin America. Many of the substances needed to make the synthetic opioid come from China and India.<\/p>\n\n\n\n<p>In American territory, its use has long assumed crisis proportions. It was magnified in the midst of the COVID-19 pandemic. In 2021 alone over 70,000 American deaths were attributed to overdoses of synthetic opioids, mainly fentanyl.<\/p>\n\n\n\n<p>Although it is called the &#8220;poor man&#8217;s heroin&#8221;, due to its comparatively low cost of manufacture, its victims come from all socio-economic strata. And indeed of any age. Even children.<\/p>\n\n\n\n<p>According to the US Drug Enforcement Agency (DEA), in 2022 more than 50.6 million fentanyl pills and more than 4500 kg of powder were seized in the US. The amount is equivalent to &#8220;more than 379 million potentially lethal doses of fentanyl,&#8221; the DEA said. But in Multnomah County\u2014home to the state&#8217;s largest city, Portland\u2014fentanyl overdose deaths increased 533 percent between 2018 and 2022.<\/p>\n\n\n\n<p>&#8220;Our country and our state have never seen a drug so lethally addictive, and we are all scrambling to figure out how to respond,&#8221; Oregon&#8217;s Democratic governor, Tina Kotek, said the other day. &#8220;In this&#8221;, he underlined, &#8220;we are all together&#8221;.<\/p>\n\n\n\n<p>That said, he jointly announced with the mayor of Portland and the chairman of Multnomah County the start of a 90-day experiment.<\/p>\n\n\n\n<p>For three months, through the end of April, downtown Portland will be in a state of emergency for the fentanyl crisis. Two-pronged, it consists of coordinating services to provide treatment and support for synthetic opioid addicts on the one hand, and stricter policing on the other.<\/p>\n\n\n\n<p>In this context a &#8220;command center&#8221; was created in Portland to coordinate the efforts. Police patrols and checks increased. A campaign to inform and raise public awareness of the dangers of fentanyl use and the addiction treatments provided was launched.<\/p>\n\n\n\n<p>The effort is also part of efforts to revive a once-thriving urban center where the fentanyl crisis fueled the economic crisis. Downtown deterioration and safety issues, combined with real estate shortages, have prompted several large businesses and residents to leave Portland in recent years. Whether the trend will reverse remains to be seen.<\/p>\n\n\n\n<p>The situation will be reassessed at the end of the emergency quarter, with the possible geographical expansion of the program. He has made innovative choices. In their application, however, they suffer.<\/p>\n\n\n\n<p>The dramatic increase in fentanyl overdose deaths in Multnomah County comes amid severe shortages in drug addiction treatment and rehab facilities in a state that has moved to decriminalize possession of small amounts of the drug by 2021.<\/p>\n\n\n\n<p>Known as Measure 110, the policy was approved in 2020 at the polls by 58.5% of voters in the state. The previous punitive approach had not worked. The only result was to fill state prisons with drug addicts. And especially from the most vulnerable social groups.<\/p>\n\n\n\n<p>The 2020 turnaround focused on funding rehab centers and new treatment programs for people with drug addiction. It was to what Oregon police would henceforth refer fentanyl users to, instead of jails.<\/p>\n\n\n\n<p>But there was no obligation to implement the measure, while the resources allocated to support drug addicts were ultimately limited. Many are now criticizing local authorities for trying to plug their gaps in the implementation of Measure 110, which is now under review.<\/p>\n\n\n\n<p>Many public health experts, scientists and academics disagree with the prospect of recriminalization.
